Sunshine Life Co – Wellness Program Liability Waiver & Release

(For Parents/Guardians of Minor Participants)

Informed Consent & Assumption of Risk

I, the undersigned parent or legal guardian, voluntarily allow my child/children to participate in Sunshine Life Co’s Wellness Program, which may include yoga, arts and crafts, self‑defense lessons, dance, and other movement‑based or creative activities offered in‑studio or outdoors.

I understand that, as with any physical or creative activity, there is always a risk of injury—ranging from minor to serious—and that such risks cannot be fully eliminated. I acknowledge that it is my responsibility to ensure my child is physically and emotionally able to participate safely in these activities.

I agree that my child will follow all instructions given by Sunshine Life Co staff and instructors. If any unusual symptoms or injuries occur during or after participation, I will ensure my child stops the activity and seeks appropriate care.

By signing this waiver, I affirm that I have disclosed any relevant health, behavioral, or safety information needed to support my child’s participation.

Release of Liability

I agree to assume all risks associated with my child’s participation in Sunshine Life Co programs. I further agree to release Sunshine Life Co, its owners, employees, contractors, and instructors, or DBA’s of the company from any and all claims, demands, causes of action, suits, or liability for injuries, damages, losses, or incidents of any kind arising from or related to my child’s participation—whether caused by negligence or otherwise.

I understand that Sunshine Life Co’s programs may be physically active and that participation is voluntary.

Communicable Disease Acknowledgment (Including COVID‑19)

I understand that participation in group activities may increase the risk of exposure to communicable diseases.
I agree that my child will not attend class if they are experiencing symptoms of illness or have had recent known exposure to a contagious condition.

If my child contracts a communicable disease through community transmission, I agree that neither I nor my heirs or representatives will hold Sunshine Life Co liable for any resulting injury, illness, or damages.

Photo/Video Release

I grant permission for Sunshine Life Co to use photographs, video, or digital media of my child for marketing, educational, or promotional purposes in print or online, without compensation. If I prefer to opt out, I will notify Sunshine Life Co in writing prior to participation.

Personal Belongings

I understand that Sunshine Life Co is not responsible for the loss, theft, or damage of personal belongings brought to class or left in vehicles.

Digital Signature

By signing below, I acknowledge that I have read and fully understand this Liability Waiver and Release. I agree to its terms voluntarily and understand that my digital or written signature constitutes a complete and unconditional release of all liability.

Cancellation & Refund Policy

No refunds are issued for missed classes or workshops. Class sizes are limited, and reserving a spot prevents another child from attending.

If you provide at least 72 hours’ notice, Sunshine Life Co may offer an account credit for a future class or event. Credits are not guaranteed and are determined case‑by‑case.

If you pre‑register for a class and cannot attend, please remove your child from the roster to open the space for another student. Repeated failure to cancel may result in a small fee.
